Applying for access to your medical record

You can request a copy of your KEMH medical record either by Information Request or under the Freedom of Information Act 1992 (external site).

 

Information Request

Patients wishing to access their KEMH medical record should submit an Information Request Form (PDF), along with a copy of photo identification, to the Freedom of Information department. There are no fees involved for information requests. Your request for access will normally be processed within 30 days (WA) of receipt.

Medical centres, general practitioners, and other hospitals who wish to access patient information should forward heir request, along with patient consent, to the Medical Records department.

The Western Australian Freedom of Information Act 1992 (external site) provides a general right of access to documents. Some documents are considered exempt under the Act and may not be released. There is no charge for an application for personal information. Requests for non-personal information incur a fee of $30 plus administrative charges. FOI applications will be managed within 45 days unless otherwise negotiated.
